MedMe's Diabetes Module is designed to support pharmacists in providing comprehensive, billable diabetes care. Built around CDE best practices, it enables both initial assessments and targeted follow-ups through guided documentation and structured care delivery.

What’s Included in the Diabetes Module

This diabetes module offers one main initial appointment type titled “diabetes initial assessment” and 8 different follow-up appointment types each with their own CDE approved documentation.

  • 1 Diabetes Initial Assessment

    • For annual diabetes-focused medication reviews.

  • 8 Follow-Up Appointment Types:

    • Foot & Eye Care

    • Heart Health Management

    • Highs and Lows of Blood Sugar

    • Insulin Therapy Management

    • New Medication Follow-Up

    • Nutrition and Lifestyle Management

    • Sick Day Management

    • Vaccinations Follow-Up

Each appointment includes curated, CDE-approved questionnaires to ensure complete, compliant documentation.

Step-by-Step: Delivering Diabetes Care

1. Schedule the Appointment

Tip: Look up “diabetes” in your appointment type selection bar to view all available appointment types associated with the diabetes module.

  • Search for "diabetes" in the Appointment Type field.

  • Choose from the Initial Assessment or follow-up appointment types.

  • Appointments can be:

    • Conducted immediately

    • Booked by sending the Scheduling Link

    • Scheduled on the patient’s behalf

2. Navigate to the Patient Profile

Access the profile via the Patients tab or from the Calendar after booking.

To get started, select the edit documentation button to open the appointment type.

3. Open Documentation

  • Click Edit Documentation.

  • Complete appointment-specific questionnaires such as:

    • Diabetes History

    • Lifestyle History

    • Medication List

    • DTPs and Monitoring

Each diabetes module appointment type is made up of unique CDE approved questionnaires to help guide each diabetes conversation and ensure complete documentation while maximizing patient benefits.

Please Note:

  • Some fields may auto-fill from the Public Intake form.

  • Additional sections should be completed in real time as needed.

4. Complete and Finalize Documentation

  • Fill in the remaining fields, including:

    • SOAP Note

    • Patient Summary

  • Address any DTPs and set follow-up recommendations.

Certain questionnaires will come pre-filled by the patient from when they booked their appointment. This ensures as much data has been collected from the patient as possible before meeting with the pharmacist.

Other questionnaires will be meant for the pharmacist and patient to go through together.

5. Download or Print Documentation

  • Click the download/print icons beside each form for recordkeeping or patient handouts.

Please Note: Each appointment generates a printable Patient Summary.

6. Book the Next Follow-Up

  • Before ending the appointment, send a Scheduling Link for the next follow-up.

  • This helps maintain care continuity and recurring billing opportunities.
