AI Scribe – SOAP Notes Documentation

This article explains how to use AI Scribe to automatically generate SOAP (Subjective, Objective, Assessment, and Plan) notes.

Follow the steps below to activate AI Scribe during your appointment, capture the conversation, and generate a complete SOAP note that you can review, edit, and save. You can also copy and paste the note into other external systems, like your EMR/PMS vendor.

Getting Started

  1. Access the Pharmacist Dashboard:

    • Log in to MedMe and access the pharmacist dashboard for your account

    • Click on the Patients tab from the main navigation, select a patient from the list or create a new patient

  2. Create a New Consultation:

    • Click the Add button to create a new appointment.

    • Search for the "Generic Pharmacist Consultation" appointment type that supports SOAP notes through AI Scribe.


Initiating a Consultation with SOAP Notes

  1. After you have selected an appointment, click the dark blue 'Edit Documentation' button to start the documentation process.


Activating AI Scribe and Capturing Audio

  1. Start AI Scribe:

    • Click the 'Start Scribe' button.

    • Confirm patient consent to start the transcription process.

    • Once consent has been confirmed, you'll notice a live wave animation indicating that audio is being captured as you or the patient is speaking.

      • (If you are not seeing any movement, ensure that the microphone is connected and turned on while also checking that the browser permission is enabled for the microphone)

  2. Conducting the Consultation:

    • Proceed with the conversation as you would do normally. AI Scribe captures the dialogue and transcribes it in real time.

    • You can pause or unpause the transcription recording at any point during the consultation.

  3. Live Transcription:

    • Watch the transcription box update live as the conversation progresses. You can select and copy/paste from the transcription section as well


Finalizing and Editing Your SOAP Note

  1. Complete the Consultation:

    • When the consultation is finished, click the 'Complete Consultation' button.

    • AI Scribe will process the recorded audio and generate a complete SOAP note with the respective sections (Subjective, Objective, Assessment, and Plan). This can take some time depending on the duration of the appointment.

  2. Review and Edit:

    • Use the rich text editor to review and adjust each section as necessary.

    • You can copy the complete SOAP note to paste into your EMR or any other documentation system if required

  3. Save Your SOAP Note:

    • Click the 'Complete' button to finalize and store the documentation.

    • You can access previous saved SOAP note content by clicking the 'Edit Documentation' button in the patient record.

  4. Feedback:

    • At the end of the consultation, you'll be prompted to provide feedback on your experience. This helps improve the AI Scribe functionality for future consultations.


Troubleshooting and Tips

  • Microphone Permissions:

    • Ensure your browser has granted microphone access. AI Scribe requires this permission to capture your conversation accurately.

  • Activation Issues:

    • If AI Scribe does not start, double-check that patient permission has been confirmed.

  • Transcription Clarity:

    • Make sure that the consultation audio is clear and that the transcription accurately reflects the conversation before finalizing the SOAP note.

  • Editing Flexibility:

    • Remember, you can always edit the SOAP note using the rich text editor if any details need manual adjustments.


Understanding the AI Scribe Components

  • Start AI Scribe Button:

    • Located prominently on the interface. Click this button to activate AI Scribe and start recording the conversation.

  • Microphone Permissions:

    • If it's your first time, you'll be prompted to grant microphone permissions in your browser. Ensure that you allow these permissions to enable audio capture.

  • Live Transcription Box:

    • Displays the real-time transcription of your conversation. This section can be expanded or collapsed as needed.

  • Rich Text Editor in SOAP Subsections:

    • Allows you to add or manually edit and format the Subjective, Objective, Assessment, and Plan sections.

  • Notes Section:

    • Use this area to add additional comments, reminders, or follow-up tasks that are not part of the formal SOAP note. Important: Customers will not see what you enter as a note on your documentation
