How to Use the MedsCheck Module in MedMe

This article explains how MedMe's MedsCheck Module works for Ontario pharmacies, covering setup, patient booking, and documentation.

About Ontario's MedsCheck Program

The MedsCheck program provides a standardized framework for pharmacists to conduct annual medication reviews. This one-on-one consultation allows pharmacists to assess a patient's ability to manage their medications, verify the appropriateness of prescriptions and dosing, and identify potential interactions, side effects, allergies, and contraindications. The program also supports communication with other healthcare providers to resolve any medication-related issues identified during the review.


How the MedsCheck Module Works

MedsCheck is a dedicated module within the MedMe system, built specifically for pharmacies participating in Ontario's MedsCheck program. It streamlines the medication review process for both patients and pharmacists.

Patient-facing features:

  • Patients can book MedsCheck consultations directly through the pharmacy's Booking Portal.

  • Patients can select their preferred Appointment Type and method (video call, phone call, or in-store).

  • During intake, patients can answer screening questions, provide contact information, and upload photos of their prescriptions or medications.

Pharmacist-facing features:

  • Pharmacists can easily find and book MedsCheck consultations directly within the MedMe system.

  • The documentation module streamlines completion of the multi-page Ontario MedsCheck forms.

  • Pharmacists can record patient consent, select the pharmacist involved, and electronically complete all questionnaire sections.

  • Drop-down menus and pre-populated options are available throughout the form.

  • A drug depository allows for quick selection of medications (prescription and over-the-counter) and allergies.

  • Once completed, the system generates a multi-page PDF summarizing the entire MedsCheck review according to Ontario program guidelines.

Tip: The completed PDF can be downloaded and uploaded directly into a pharmacy's Pharmacy Management System (PMS) for easy record keeping.

MedsCheck Module Workflow

Patient-Facing Workflow

  1. If Public Intake is enabled for the MedsCheck Appointment Type, patients can visit the pharmacy's Booking Portal to book a MedsCheck consultation.

    Please Note: The Appointment Type's description can be customized from within the system.

  2. Patients select their preferred consultation method: Video (HIPAA-compliant), Phone, or In Store.

  3. Patients select an available date and time for the consultation.

  4. Patients complete the intake form, which includes:

    • Contact information

    • Medical information (e.g., health card number, family doctor)

    • Optional photo upload of prescriptions or medication vials

    • Consent confirmation

    • A final review of all submitted information

  5. Once submitted, patients are directed to the appointment confirmation page.

    Reminder: The "Next Steps" messaging and notification wording can be edited from the Details tab within the MedsCheck Appointment Type settings.

  6. Patients receive SMS and email notifications confirming their consultation details.


Pharmacist-Facing Workflow

Pharmacists can also book a MedsCheck consultation for a patient directly within the MedMe system, using either the Patients tab or the Appointments tab.

Booking MedsCheck from the Patients Tab

  1. Select the patient from the list.

  2. Click Create.

  3. Select MedsCheck Annual from the Appointment Type dropdown.

  4. Select the method, duration, and calendar.

  5. Choose who is booking the Appointment Type.

  6. Enter the start date and time.

  7. Click Schedule Appointment.

Booking MedsCheck from the Calendar/Appointments Tab

  1. Click Create/Add.

  2. Select MedsCheck Annual from the Appointment Type dropdown.

  3. Select the method, duration, and calendar.

  4. Type the patient's name and select it from the dropdown.

  5. Choose who is booking the Appointment Type.

  6. Enter the start date and time.

  7. Click Schedule Appointment.

For Instance: A pharmacist booking on behalf of a walk-in patient would typically use the Appointments tab rather than searching the Patients tab first.

Starting MedsCheck Documentation

  1. Navigate to the patient's profile and select their MedsCheck consultation.

  2. Record the patient's consent if they have not provided consent yet (appointment was not booked through the booking page).

  3. In the documentation pop-up, click Start Scribe to begin the clinical assistant transcription. Click on confirm after you have obtained consent to transcribe the consultation.

  4. Complete the multi-page MedsCheck questionnaire. This form is pre-populated within the system, reducing time spent compared to traditional paper forms.

  5. After completing the consultation, click Pause Scribe to stop the transcription then populate the fields (if you are using AI Scribe).

  6. Review the documentation.

  7. After reviewing, click Complete.

  8. Allow the system a few seconds to generate the final multi-page Ontario MedsCheck PDF documentation.

  9. Once generated, print or download the documentation as needed.
