Booking an Appointment as a Patient

Patients select appointment time then complete the intake forms. Patients also answer when they are travelling. These details will be recorded on both the auto-generated lab requisition and the patient's profile to help pharmacists assess testing eligibility and review identification.

Lab Requisition Generation

STEP 1: Ensure patient information is accurate. Change the information by clicking the Edit button.

STEP 2: Fill out the required information in the pop-up after pressing Edit. Press the green Update pdf button to save all the information.

STEP 3: Document the pharmacist (or other healthcare provider) that administered the test. Input the pharmacist name and license number if the test administrator is someone other than the logged in account.

STEP 4: Use the Print/Download button to save the file to the computer. Then print the pdf or send electronically to the lab
