Getting Started
AI Scribe listens during a Consultation and automatically drafts a SOAP note (Subjective, Objective, Assessment, and Plan), reducing the time pharmacists spend on manual documentation.
Accessing the Patient Record
Log in to MedMe and access the Pharmacist Dashboard.
Click the Patients section from the main navigation.
Select an existing Patient from the list, or create a new patient record.
Initiating a Consultation with SOAP Notes
Click the Add button to create a new appointment.
Search for and select the SOAP Note Appointment Type, which supports documentation through AI Scribe.
Once the appointment is created, click the Open button in the Documentation panel to begin the documentation process.
Reminder: Only Appointment Types configured to support AI Scribe will display the SOAP note documentation workflow.
Activating AI Scribe and Capturing Audio
Click the Start Scribe button.
Confirm Patient consent before transcription begins.
Once consent is confirmed, a live wave animation will appear, indicating that audio is being captured.
Warning: If no wave animation appears, confirm that a microphone is connected and turned on, and that microphone permissions are enabled in the browser.
Proceed with the consultation as usual. AI Scribe transcribes the conversation in real time.
Transcription can be paused or unpaused at any point during the consultation.
Review the transcription box as it updates live throughout the conversation.
Text in the transcription section can be selected and copied for reference.
For Instance: A pharmacist may pause AI Scribe briefly if a patient steps away mid-consultation, then resume once the conversation continues.
Finalizing and Editing the SOAP Note
When the Consultation is complete, click the Pause Scribe button to stop the transcription.
Click on either Populate Fields or Populate All Questionnaires for AI Scribe to process the recorded audio and generate a complete SOAP note, organized into four sections: Subjective, Objective, Assessment, and Plan.
Use the rich text editor to review and adjust each section as needed.
The completed SOAP note can be copied (using the copy icon next to the appointment type name) and pasted into an EMR/PMS or other external documentation system.
Click the Continue button to finalize and save the SOAP note.
Previously saved SOAP notes can be accessed at any time by clicking Open in the Patient record.
After saving, provide feedback on the AI Scribe experience when prompted.
Please Note: Feedback submitted after each consultation helps improve AI Scribe accuracy over time.
Starting AI Scribe from Quick Actions
Quick Actions offers a faster way to start AI Scribe without navigating through the full appointment flow.
Click Quick Actions.
Select Start Scribing.
Choose the Appointment Type, in this case a SOAP note.
Click Proceed to open the documentation card.
Click Start Scribe and follow the directions as stated above.
Understanding the AI Scribe Components
Start AI Scribe Button: Activates AI Scribe and begins recording the consultation.
Microphone Permissions: On first use, the browser will prompt for microphone access. This must be allowed for AI Scribe to capture audio.
Live Transcription Box: Displays the real-time transcription of the Consultation and can be expanded or collapsed as needed.
Rich Text Editor: Allows manual editing and formatting of the Subjective, Objective, Assessment, and Plan sections.
Notes Section: Used to add internal comments, reminders, or follow-up tasks that are not part of the formal SOAP note.
Please Note: Content entered in the Notes section is internal only — Patients will not see notes added here.
Troubleshooting and Tips
Microphone permissions are not enabled: Confirm that the browser has been granted microphone access. AI Scribe requires this permission to capture the Consultation accurately.
AI Scribe does not start: Double-check that Patient consent has been confirmed before starting the recording.
Transcription is unclear or inaccurate: Confirm that the Consultation audio is clear before finalizing the SOAP note. Background noise or overlapping speech may affect transcription accuracy.
Need to make manual corrections: The rich text editor allows the Subjective, Objective, Assessment, and Plan sections to be edited manually at any time before saving.
